How to become a Pain Clinic Patient

The requirements for becoming a patient at a chronic pain include:

  1. Referral from a primary care physician or specialist
  2. Diagnosis of chronic pain: Patients must be diagnosed with chronic pain, which is defined as pain that lasts for more than three months.
  3. Completion of a patient intake form: Patients are required to complete a detailed intake form that includes information about their medical history, current symptoms, and treatment goals.
  4. Medical tests and exams: Patients are required to undergo medical tests and exams, such as blood tests, X-rays, or other imaging studies, to help the clinic diagnose the underlying cause of their pain and develop an appropriate treatment plan.
  5. Patient agreement: Patients are required to sign a patient agreement, acknowledging that they understand the risks and benefits of the treatments being offered and have read the clinic’s policies and procedures.
